What is MyChart?

MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to:

  • Request medical appointments.
  • View your health summary from the MyChart electronic health record.
  • View test results.
  • Request prescription renewals.
  • Access trusted health information resources.
  • Communicate electronically and securely with your medical care team.

How do I sign up?

Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code enables you to log in and create your own user ID and password or you can sign up on the Web site by clicking on the Sign Up with Activation Code Now link. If you were not issued an activation code in the clinic, click on the Request an Activation Code link.

When can I see my test results in MyChart?

Most test results are automatically released to your MyChart. This means the ordering physician may not have had an opportunity to review them. Please allow time for the physician to review your results, and to send you comments if needed. Test results will not be viewable if they have not yet been released, or if they are sent to us on paper. All results that fall within our release criteria will be available upon activation of your MyChart account. Not all results that have a scanned linked attachment will be viewable in MyChart at this time.

Why are certain test results not shared electronically via MyChart?

Certain tests may not be released through auto-time release, these tests include those subject to restrictions by California law and tests ordered for research purposes. Your provider is able to determine which types of test results are available through MyChart.

If some of my health information on MyChart is not correct, what should I do?

MyChart information comes directly from the electronic medical record at your doctor's office. Send a message to your doctor's office via a "MyChart Advice Message" regarding any information that needs to be updated or ask your doctor to correct any inaccurate information at your next clinic visit. Health information is reviewed and updated at each visit.

If I send a message to my Care Team, when can I expect a reply?

The Care Team includes your primary care physician and the specialists you may see, plus the nurses and office staff who support them. You will generally receive an answer within 2 business days. Please note that MyChart should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

Why isn't my provider listed when trying to Message My Doctor?

Some providers are not listed in MyChart including Radiologists, Psychiatrists, Residents, and Pathologists, InPatient/Hospital Specialists etc. Generally, providers you come into contact with during your hospital stay will not be available to message in MyChart. If you do not see the provider listed that you are attempting to message please call the appropriate clinic to address your current concern.

Can I view a family member's health record in MyChart?

Yes, accessing your family's record allows a patient's legal representative (spouse, parent, guardian, conservator) to log into his/her personal MyChart account and view the patient's medical information provided in MyChart. Complete an Access to My Family's Records Consent form, available on this website. Patient representatives must be at least 18 years of age. UC Davis offers parents/guardians medium proxy access. Features include messaging the Care Team, Immunization Summary, and Allergies. Coming soon we will offer teen access.

PLEASE NOTE: Patient representatives must be at least 18 years of age. In accordance with California State confidentiality laws specific to teens, proxy access converts to a medium access on the child's 12th birthday and proxy access terminates on the child's 18th birthday.

Birth to 12 Years of Age: Parents and legal guardians are provided with full access to their child's MyChart account. Fifteen days before your child's 12th birthday, you will receive a notification alerting you that your access will be transitioned to a medium proxy access the day before your child's birthday.

12 to 17 Years of Age: Parents and legal guardians are provided with medium access to their child's MyChart account. Fifteen days before the child's 18th birthday, you will receive a notification alerting you that access will be terminated the day before your child's birthday.

Can I ask questions regarding a family member from my MyChart account?

No. MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

Can I obtain a MyChart account for my family member if I do not receive my health care at UC Davis?

Yes, you can receive a MyChart account for your family member.

Can my spouse and I share one MyChart account?

No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.

Is MyChart access available for patients 12 years old to 17 years old?

Patients age 12-17 may use MyChart to:

  • Message their care team
  • Request an appointment
  • Review allergies, immunizations and growth charts
  • Review helpful and trustworthy health information on many topics, from acne to birth control
  • By law, teens are able to obtain some kinds of health services or information without notifying their parents or guardians. In MyChart, teens must take specific action to prevent their parents with proxy access from seeing these kinds of online messages or appointment requests.     Can you send me a new activation code as I have lost it, let it expire or did not receive it?

    Can you send me a new activation code if I lost it, let it expire, or did not receive it?

    Yes, if the original activation code has never been used, click Request An Activation Code, complete the form, and then after we verify your information, a new code will be sent via e-mail. If the original activation code has been used, click Forgot MyChart Username and complete the form. Your MyChart Username will be emailed to you.

    Where can I update my personal information (e.g., home address, e-mail or change my password)?

    Log in to MyChart. From the left menu, click "Preferences" to make updates to your MyChart account. Update your provider's office directly with a change of address and phone number.

    How is MyChart secure?

    We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. Further, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.

    What is the UC Davis Health System's Privacy Policy?

    MyChart is owned and operated by UC Davis Health System and is fully compliant with federal and state laws pertaining to your privacy. We treat your name and e-mail address with the same care and privacy given to your health records. No information will ever be sold or leased by MyChart.

    I was logged out of MyChart, what happened?

    We aim to protect your privacy and security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

    Is my information stored on the Internet?

    No. We take your privacy very seriously. When logged into MyChart, the Web servers request data from a secure database and present that data over a SSL (secure sockets layer) connection to ensure confidentiality. At no time is this data stored on the Web servers or made available to search engines.

    My activation code does not work, what should I do?

    For your security, if you do not activate your MyChart account within 45 days, your activation code expires and is no longer valid. If this occurs, click Request an Activation Code, complete the information, and click "Submit." After we verify your information, a new code will be sent via e-mail.

    Is my activation code my username?

    No, your activation code is not your MyChart username or password. Use the activation code once to log into MyChart for the first time. (The code expires after use or after 45 days). During your first visit to MyChart, you create a unique MyChart username and password.
